Birth & Death Certificates

    

Connie Shiverdecker, Deputy Registrar, has been employed with the Health Department since 2007. She issues birth and death certificates, collects vital statistics, and prepares minutes for the Fatality Review Board. She also assists with other secretarial duties within the environmental division.
 

 The Darke County Health Department has birth and death certificates for individuals that were either born or died in Darke County.

To obtain a copy of a certificate of a birth or death that occurred in another county, contact that county.

Records start December 20, 1908 to the present.

Anyone wishing to deliver their baby at home can pickup the birth forms that need to be filled out.

We have Certificates of Service – people who died in another state but are buried in Darke County.

The fee for certified copies of birth or death certificates is $18.00

Issuing of burial permits $3.00
