Nursing Services
The Community Health Nursing Services Division is responsible for meeting
the collective health needs of the citizens of Darke County by identifying
health problems and managing interactions around those identified needs.
The goal is to protect and promote the health of all citizens of Darke
County through assessment, development of programs to meet the identified
needs, and assure that the programs developed actually meet the needs.
This is general and comprehensive nursing practice, which includes individuals,
families, and groups, with the dominant responsibility to the population
as a whole.
Services:
Car
Seat Programs
Cosco Convertible Car Seats: Parents of children needing a car seat may
obtain a new Cosco Convertible Car Seat at the Darke County Health Department.
The convertible car seat is for a child weighing 10-30 pounds with a height
between 19-40 inches. These car seats are provided by the State of
Ohio under the Ohio Buckles Buckeyes Program.
Eligibility Requirements:

Must have need for convertible or high back booster seat – not to
be used for second vehicle.

Be WIC eligible or meet WIC income guidelines.

Show WIC coupons or last 3 pay check stubs.

HELP ME SLEEP
The
mission of the Help Me Sleep program is to prevent SIDS (Sudden Infant Death
Syndrome) and numerous other ways that children can become seriously injured or
even die from unsafe sleep environments.
Eligibility Requirements: Families who are at 185%
poverty level (qualifying for WIC, Metropolitan Housing, Medicaid, SSI or
Public Assistance) and active with the Help Me Grow program, will be screened
for their need of a crib. Then, they must agree to a one-hour class about safe
sleep habits before receiving a free, safety approved crib.
All
cribs distributed will be purchased strictly from
donations.

2007 FLU
IMMUNIZATIONS
Help me grow– is a program for Ohio’s expectant parents, newborns,
infants, and toddlers that provides health and developmental services so
that children start school healthy and ready to learn. It is a consolidation
of three programs: Welcome Home Baby, Ongoing, and Early Intervention.
Eligibility Requirements:

Newborns, infants, and toddlers under the age of 3 and their families
Services Provided:

1-800 phone number that gives referrals to families

Answers to questions about infant and toddler’s health and well
being
-
Support and information about infant or toddler’s immunizations,
nutrition, and development.

Offers and brings together special services such as prenatal visits,
home visiting and support to families and children.

Helps eligible families obtain diagnostic and treatment services for children.

Communicable Disease Control
Reportable Diseases
The Health Department investigates outbreaks of reportable diseases as
required by the Ohio Revised Code. Information and counseling are
provided as needed. Classes for Day-Care providers are available
upon request. Contact the health department for additional information.
Tuberculosis Control Program
The Darke County Health Department investigates confirmed and suspected
cases of tuberculosis, including contact and skin testing as appropriate
to prevent spread of the disease. Contact Dennis Wein, RN, PHN, at 937-548-4196,

Sexually Transmitted Diseases
The Darke County Health Department provides information on sexually transmitted
