The Darke County Health Department is located within the County Government Building within Greenville City Park at 300 Garst Avenue, Greenville, Ohio. The Nursing Division is located on the top floor, and the Environmental Division is located in the basement.

Taking State Route 121 South into Greenville: At first 4-way stop sign, make a right onto Main Street. Main Street to first stop light (get in Left lane). Go Left through the light (continuing on Main Street), get in Right lane and turn Right at next light (almost immediately) onto Park Drive. Park Drive to 4-way stop sign. Turn Left. Follow the road along creek (Wilson) to a large 2 story Brick Building (Government Center).
If taking State Route 118 from Ansonia into Greenville, you will pass two stoplights (first one at Russ Road and the second at Harmon Drive) as you come into Greenville. Garst Avenue will be the third street after the second stoplight. Make a left on to Garst Avenue, continue one block, and Government Center will be on the left.
Taking State Route 127 South into Greenville, take St. Rt.121 South Exit into Greenville. Go through stoplight (intersection of St. Rt.121 and Russ Road). At first 4-way stop sign, make a right onto Main Street. Main Street to first stop light (get in Left lane). Go left through the light (continuing on Main Street), get in Right lane and turn Right at next light (almost immediately) onto Park Drive. Park Drive to 4-way stop. Turn Left. Follow the road along creek (Wilson) to a large 2-story Brick Building (Government Center).
If taking State Route 571 West into Greenville, State Route 571 joins with US 36 at the intersection of State Route 127 and US36 in Greenville. Make a right onto Ohio Street, which is at the first stoplight as you come into Greenville. Continue North on Ohio Street until you come to a stop sign. Make a left onto Main Street. Main Street to first stop light (get in Left lane). Go Left through the light (continuing on Main Street), get in Right lane and turn Right at next light (almost immediately) onto Park Drive. Park Drive to 4-way stop sign. Turn Left. Follow the road along creek (Wilson) to a large 2 story Brick Building (Government Center).
Take State Route 121 North into Greenville from New Madison until you come to the traffic circle. At the traffic circle, take State Route 118 North (North Broadway). At three-way stop, follow State Route118 north approximately 2 blocks. Make a right onto Garst Avenue. Continue 2 blocks, and Darke County Government Center will be on the left.
If taking State Route 502 East into Greenville, at first stop sign as you come into Greenville, make a left onto Main Street (State Route 121). At the traffic circle, take State Route 118 North (North Broadway). At three-way stop, follow St. Rt.118 North approximately 2 blocks. Make a right onto Garst Avenue. Continue 2 blocks, and Government Center will be on the left.
If taking State Route 571 East from Union City into Greenville, at first stop sign (which is a three-way stop), make a left turn onto State Route 118 (North Broadway). Continue 2 blocks and make a right onto Garst Avenue. Continue on Garst Avenue 2 1/2 blocks and Government Center will be on the left.
If taking US Rt. 36 East, take US Rt. 127 North. Exit at the US Rt. 36/ State Route 571 exit. Take this West into Greenville. At first stop light, make a right onto Ohio Street. Continue North on Ohio Street until you come to a 4-way stop sign. Make a left onto Main Street. Main Street to stop light (get in Left lane). Go Left through the light, (continuing on Main Street), get in Right lane and turn Right at next light (almost immediately) onto Park Drive. Park Drive to 4-way stop. Turn Left. Follow the road along creek (Wilson) to a large 2 story Brick Building (Government Center).
